- A brief historic review of theories regarding determinants of disease, with emphasis on the area of child health
- A discussion of how current knowledge may contribute to integrate distal (political, social, economic), intermediate (behavioral, environmental) and proximate (microbiological, chemical, immunological, etc) determinants in hierarchical models of causality

A discussion of inequities in child health and how these may be mediated trough intermediate and proximate factors
- A discussion of the life-course approach to determinants of health, with some counter-intuitive examples of how risk factors for child health may represent protective factors in adulthood
