The process of Health Policy Reform in the English-Speaking Caribbean: Case studies in National Health Insurance
Abstract: It documents and analyses the process of Health Policy Reform in the English-speaking Caribbean. It focuses on the development of policies for National Health Insurance in three countries in the Region: Trinidad and Tobago, St Lucia and St Vincent & the Grenadines. The study is unique, as it provides the first documentation of the policy process for National Health Insurance Programmes in the English-speaking Caribbean, thereby adding to the literature on Health Policy Process. The research was conducted (1996-2002 part-time) in accordance with the degree requirements for Doctor of Philosophy (PhD) at the University of Leeds, Nuffield Institute for Health, UK.

Reproductive health and health sector reform in developing countries: establishing a framework for dialogue
This paper examines factors that limit dialogue and proposes a framework for encouraging it, including the identification of policy groups, development of bases for collaborative links; introduction of a common understanding around relevant policy contexts; reaching agreement on compatible aims, developing causal links between policy content in reproductive health and health sector change as a basis for evidence-based policy-making. Participatory Evaluation - Final Report

Nepal Legalizes Abortion
(March 14th - Kathmandu) Reversing 150 years of legal discrimination against women, Nepal’s Lower House of Parliament passed an amendment to the Civil Code that partially legalizes abortion and brings about sweeping changes in many other discriminatory laws. In a nation where one in five women in prison have been incarcerated for having an abortion, Nepal’s government now must take action to release those women. Hepatitis B Vaccination In The Colombian Amazon. Effectiveness And Factors Influencing Vaccination Coverage
Abstract: A vaccination coverage survey was carried out in the Colombian Amazon, a former high endemic area for hepatitis B, involving 3573 children less than 11 years old. It was carried out in Leticia, Puerto Nariño, and Araracuara, both urban and rural areas. In conclusion, the introduction of a recombinant Cuban manufactured hepatitis B vaccine has produced a marked decline in the high infection prevalence of children in the Colombian Amazon area.

Measuring socioeconomic status/position in studies of racial/ethnic disparities: examples from maternal and infant health.
Conclusions about the role of race/ethnicity (and SES) in health could vary with how SES is measured. Education is not an acceptable income proxy among ethnically diverse childbearing women. Based on theoretical considerations and these and previous examples, SES measures should generally be outcome- and population-specific, and chosen on explicit conceptual grounds; researchers should test multiple theoretically appropriate measures and consider how conclusions might vary with how SES is measured.

The Impact of HIV/AIDS on Social Safety Nets: What Can the Caribbean Expect?
Estimates for the Caribbean highlight the magnitude of the potential economic burden of the disease for this region, showing its capacity to erode the economic base and by extension, and the need for nations to arm themselves for what may well be the biggest health and development battle that the region has engaged in to date. Information on the extent of the economic losses expected from HIV/AIDS has since been used as one of the critical inputs into the advocacy efforts in the region. Working Group on Utilization of Scientific Information
Difficulties exist in the utilization of scientific evidence in guiding policymaking, which have puzzled researchers and decision-makers alike. In a series of case studies done in Mexico, Bronfman et al. identified four main types of obstacles in the use of health research by decision-makers, i.e. obstacles related to context, content, stakeholders, and process. Appropriate means of diffusion of research results, which is one element that this workshop, promoted by PAHO and FUNSALUD addressed.

The purpose of the study is to generate empirical knowledge that will provide policy makers with an understanding of the influence of insurance coverage on demand for maternal health care. Growth is Good for the Poor
This study investigates link between the income of the poor (defined as the bottom 20 percent of the income distribution) and overall income (per capita GDP) using data from 80 countries over 40 years. The study examines the poverty-growth relationship in cases of poor countries versus rich countries, crisis periods versus normal growth periods, and the recent period compared to earlier times. Nutritional Equity and National Food and Nutrition Security Programs: The Case Study of Jamaica (Tecnical Report Series, No. 73)
The purpose of this study is to analyze the national food and nutrition programs in the Caribbean Region as part of public policy analysis. Poverty is defined as a state whereby individuals and groups are unable to satisfy basic needs such as food. The analytical approach used in this study combined focus group participation and nutritional data to illustrate (a) whether these programs are alleviating the effects of poverty and (b) if nutritional equity is achieved by access to food, utilization and financing a minimum nutritional package of food at the national intervention level. The methods: A sub-sample of 93 children from a 1982 epidemiological birth cohort was selected randomly from eight census districts. Unstructured methods (participant observation, unstructured yet guided interviewing and semi-structured observation) were used repeatedly with each adolescent from December 1997 to August 1998. While it is widely recognized that breastfeeding is best for babies, many women stop breastfeeding earlier than recommended because they believe they are producing insufficient milk. There is a debate about whether women who report insufficient milk actually produce less breastmilk, or whether the perception of milk insufficiency results from factors such as cultural beliefs, misconstrual of the breastfeeding process, or inadequate social support.

Health System Inequalities and Inequities in Latin America and the Caribbean: Findings and Policy Implications
The paper summarizes the concepts, methods, and results of a study on measuring health system inequalities in six countries of the Latin American and Caribbean region: Brazil, Ecuador, Guatemala, Jamaica, Mexico, and Peru. Illness concentration curves and a health status inequality index are used to assess and compare health status inequalities. Concepts of vertical and horizontal equity and inequity and inequality indices are used to measure inequities in access to health care services and in the financing of national health systems. Benefit incidence analysis is used to assess the distribution of the benefits of government expenditures on health by socioeconomic groups.

Economic Growth and the Income of the Poor
This paper examines the factors associated with growth in the incomes of the poor across countries. Aggregate economic growth is highly correlated with the growth in the incomes of the poor. On average across countries, an increase of one percent in average income (both level and growth rate) corresponds to an increase of one percent in income of the poor. While there are some countries in which the incomes of the poor grow more slowly, there are an equal number in which the incomes of the poor grow faster than overall growth.CAER II Discussion Paper 36. Progress report of the multicenter project on violence (Project Activa)
Violence in the Region of the Americas has been a source of concern among different social sectors, including governmental and non-governmental organizations. In addition, violence has been defined as a major source of worry in population surveys among residents of large cities. Violence is one of the greatest threats to public health and social security, since it is not only the cause of numerous premature deaths, but also a cause of injury and disability.

Progress report on multicenter projects
Project ACTIVA ended its data collection phase in April 1997 and began compilation of the regional database and the corresponding clean-up and standardization of the data for comparative analysis. It was a laborious process, and a totally clean database was ready by January 1998.

During the present biennium 1998-1999, HSP will address among others priorities, strengthening HSSR as the Restructuration Directive dictates. It is timely to do it because the conceptual SSS cooperation framework is defined. The fundamental proposition is that the HSSR the Organization promotes be used to strengthen the SSS cooperation areas in the four programs of the Division.

Nineteen ninety-six could be characterized as a year of transition, in which modalities for the receipt and financing of research proposals existed side by side with unsolicited proposals and new modalities for the promotion and support of research, such as research competitions, programs for the training and education of investigators in public health, and support for multicenter projects.

Nurses making the difference: WHO Collaborating Centers for Nursing and Midwifery Network (ACHR32/97.10)
The nursing response to global health is to hold governments accountable by assuring visibility of issues that count, by initiating meaningful debate, and by proposing solutions for change. This common cause has strengthened the international cohesiveness of the nursing community to focus on the formation of partnerships with groups to promote health and prevent diseases.

Modern medicine, the outgrowth of the fundamental contributions of Pasteur, Jenner, Koch, and other researchers, has developed immunization techniques, treatments, surgical procedures, pregnancy, childbirth and postpartum care, and improvements in environmental sanitation. These have changed the course of diseases and health problems.

Estimating Household Health Expenditures
In most developing countries household health spending accounts for the largest single source of financing entering the health sector. Accurate estimation of this critical part of the sector's funding base is what distinguishes National Health Accounts (NHA) from other more conventional health expenditure studies, and is the one that presents, typically, the greatest problems in estimation.

Mental Health legislation in the Caribbean (Technical Report - Series #2, December 1991)
This paper analyzes the mental health legislation of the English-speaking Caribbean to determine if it conforms to current international standards for such legislation. The topics discussed were mental health as a public health problem, third world mental health care: conventional institution model, the law and mental health and the Caribbean mental health legislation. It was concluded that mental health legislation in the Caribbean is generally antiquated and makes no allusion to psychiatric service delivery, community-based services, national mental health policy, standards of care and treatment, designation of authority for mental health care, and provision of patient rights. But vast potential for legislative modernization exists in the islands.
