County Health ranking | Ranking methods
Interactive map for the spatial distribution of County Health Ranking (2nd admin level). The Rankings are based on a model of population health that emphasizes the many factors that, if improved, can help make communities healthier places to live, learn, work and play. Building on the work of America's Health Rankings, the University of Wisconsin Population Health Institute has used this model to rank the health of Wisconsin’s counties every year since 2003.

Health Indicators Warehous (HIW)
The purpose of the HIW is to: provide a single, uiser-friendly, source for national, state, and community health indicators; meet needs of multiple population health inititatives; facilitate harmonization of indicators across initiatives, link indicators with evidence-based interventions; and serves as the data hub for the HHS Community Health Data Inititative, a flagship HHS open government initiative to release data.
