PAHO/WHO Passport to Healthy Lifestyle (2014)
The Chronic Care Passport is a useful tool to care of chronic diseases in an effective way. It consists of a booklet carried by the patient with chronic conditions such as diabetes, hypertension and chronic obstructive pulmonary disease.
The Chronic Care Passport was designed for the first level of health care, but can be used in other settings. It arose from PAHO's technical collaboration with several Member States.
