Across the Caribbean, climate change is intensifying health risks and placing new pressures on the systems and services on which people rely. Turning regional commitments into tangible results requires more than a shared vision: it requires clear measures of progress, consistent reporting, and evidence that action is reaching the people, communities, and health systems most affected. This publication accompanies the Caribbean Action Plan on Health and Climate Change 2026–2031 and provides standardized guidance for monitoring its implementation. It presents detailed descriptors for 22 indicators organized around the Action Plan’s four Strategic Lines of Action: Governance and Leadership; Climate Resilient Health Systems (Adaptation); Low Carbon Health Systems (Mitigation); and Resources and Financing. For each indicator, the publication explains what is being measured, why it matters, which criteria must be met, and what evidence can be used to demonstrate progress. The indicators address essential areas of action, including multisectoral governance, workforce capacity, the integration of health into national climate policies, early warning and climate-informed surveillance, continuity of essential health services, research, health-sector greenhouse gas measurement and reduction, and access to sustainable climate financing. By establishing common definitions and reporting expectations, the indicator descriptors support greater consistency and comparability across Caribbean countries and territories while recognizing differences in size, capacities, and institutional arrangements. The framework enables countries to identify gaps, document achievements, and prioritize the next steps needed to strengthen climate resilient, low carbon, sustainable health systems and address health inequities. It also supports regional partners in targeting technical cooperation and resources where they can have the greatest impact. Designed for ministries of health, national climate and health focal points, and regional partners, this publication is both a monitoring resource and a practical guide for implementation.
