Context

HEARTS in the Americas is a regional program led by the Ministries of Health of 33 countries, with the participation of other stakeholders and the technical cooperation of PAHO. This program serves as a spearhead for the implementation of the PAHO initiative "Better Care for NCDs," which seeks to expand equitable access to integrated and comprehensive services for the most prevalent NCDs in PHC settings, strengthening countries' capacity to plan, implement, and offer quality management for these conditions.

The Clinical Pathway is the primary implementation tool for HEARTS in the Americas and, to date, has been adopted by 29 countries in the region as the standard for hypertension and cardiovascular risk management in PHC. This clinical pathway is a living instrument that can evolve to incorporate new evidence, expand its scope, and strengthen health teams' capacity to provide higher-quality care.

HEARTS 2.0 is a project born within HEARTS in the Americas, with the objective of leading an effective transition from the isolated control of hypertension toward the integrated management of Cardio-Kidney-Metabolic Syndrome, based on the best available evidence. The result is an improved clinical pathway model that will serve to strengthen the clinical pathways of the countries implementing HEARTS in the Americas, optimizing the health outcomes of the population.
